Geriatric Care Managers
Looking for a Geriatric Care Manager to perform assessments on new clients, create care plans, implement care, and train new employees to implement these care plans.
Works with Nursing Supervisor to develop and implement new MyPartners In Living Programs to achieve optimum health Care results for our clients.
Prefer at least 5 years experience as a Geriatric Care Manager, RN degree preferred.
